Patient Disability Forms

PLEASE NOTE: There is a $25.00 processing fee for new form(s) and a $10.00 processing fee for updated form(s). We have a rush request available with a $50 charge. You have to request the form within 48 hours in the Comment section for us to charge the $50 fee. In order to have your form(s) processed, pre-payment is required. The Carrell Clinic will make every effort to quickly process and return your form(s) to you but can take up to 15 days depending on the circumstances.

Want to Get Your Forms Completed Faster?

Completing the online form and submitting your disability forms online will greatly speed up the process of getting your forms returned to you. There is no need to drive up to the clinic when you can simply upload your form and submit it for processing. Once your form is received, a Carrell Clinic staff member will call you to receive payment. Once payment is received, your forms will be processed.
